Please bring with you to your appointment:

1. All dental/medical insurance cards covering the patient to be treated

Please note: Failure to have all dental insurance info could delay or postpone your treatment!

        Per your insurance company, we will need the following information to properly bill your insurance:

                Subscriber name, Subscriber date of birth, Subscriber's SSN

2. Photo ID of the responsible party on the account

3. X-rays from your referring dentist (if applicable)

4. Complete list of all medications you are currently taking including non-prescription medications plus any allergies

5. Patient's Primary Care Physician's name and telephone number

6. Form of payment

7. A driver with Current Driver's License

8. Empty Stomach (please do not eat or drink - including water - for at least 6 hours prior to your appointment)
